- You have been diagnosed with diabetes and are treated with insulin.
- You have a history of hypoglycemia with documentation of either:
- one or more hypoglycemic events (level 2 hypoglycemia)
- one hypoglycemic event (level 3 hypoglycemia)
- Your doctor has determined that you (or your caregiver) have adequate training using a CGM and has written a prescription.
- You have an in-person visit within 6 months of starting CGM and every 6 months thereafter.
- Your CGM is being used as indicated by the FDA.
